The results of the self-assessment remain valid for the duration of the day that the screening was completed, until 11:59 pm. If passed, the result is no longer valid if your situation changes during the day (i.e. you start experiencing symptoms of COVID-19).

Please do not enter the workplace (including any outdoor, or partially outdoor, workplaces). If you are experiencing symptoms of COVID-19 or have come into contact with a confirmed or probable case, go home to self-isolate immediately and contact your health care provider or Telehealth Ontario (1 866-797-0000) to find out if you need a COVID-19 test.
You can proceed to the MPAC office.

Keep a distance of at least 2 metres from people you do not live with, wear a face covering or mask while indoors that covers your mouth, nose, and chin, and wash your hands often.
Do not enter the MPAC office.
Based on your answers, you either have symptoms or may have been exposed to COVID-19. We recommend that you stay home to protect the health and safety of the people you work with. You could be carrying or spreading the virus without knowing it.
Let your MPAC contact know about this result, and speak with a healthcare provider or Telehealth Ontario (1-866-797-0000) to get advice or an assessment, including if you need a COVID-19 test.
